York State Department of Health as a Primary Stroke Center, with

have an exciting opportunity to join our team as a PD - Physical
Therapist. In this role, the successful candidate Evaluates, tests
and treats inpatients as per referral. Develops comprehensive
therapeutic plans, documents patient progress daily, revises
treatment plans when appropriate, educates patient on disease
process and is involved with discharge planning. The therapist uses
all physical therapy modalities and techniques. Provides lectures
and demonstrations to the hospital staff and affiliations as
requested. Job Responsibilities: 1. Clinical Responsibilities. •
Covers all Physical therapy areas. Identifies clinical limitations
and seeks guidance as necessary to address limitations. •
Voluntarily helps other staff members. • Assists in the supervision
of Physical Therapy Assistants, the Rehabilitation Aide and the
Rehabilitation PSA. • Initially and ongoing, reviews medical
record, observes and interviews patients, reviews medical histories
including physicians’ diagnoses and orders. • Performs and
documents all initial evaluations and progress notes in a timely
fashion. • Practices in a safe manner that minimizes risk to
patient, self and others. Demonstrates awareness of
contraindications and precautions of treatment. • Selects
intervention strategies to achieve the desired outcomes. • Adjusts
the plan of care in response to changes in patient status. •
Establishes short and long term goals in conjunction with the
patient and formulates treatment plans consistent with these goals.
• Provides patient and families with education in regards to
disease process and therapeutic intervention to achieve goals.
Modifies educational activities considering learner needs,
characteristics and capabilities. • Determines the appropriate
equipment and modalities required to achieve goals. • Coordinates
physical therapy with other services to facilitate efficient and
effective patient care. • Discusses treatment plan and/or
associated problems with physicians and nursing staff. • Determines
physical therapy related tasks that can be legally and ethically
delegated. • Follows the established Policies and Procedures and
Safety precautions in the use of all modalities • Examines
equipment prior to patient use. • Organization of workload to meet
deadlines and priorities established by management. 2.
Documentation: • Legibly documents in the patient’s chart, on
departmental forms, and any other hospital forms all aspects of
Physical Therapy care. This is to include screening examination,
evaluation, response to treatment, discharge planning, family
conferences and communication with other health care professionals.
• Selects relevant information to document the delivery of physical
therapy patient care. • Documents any patient or family education
on the hospital Patient and Family Education form. • Fully
completes departmental forms such as the Physical Therapy Initial
Evaluation form and Physical Therapy re-evaluation form. • Produces
documentation that follows guidelines and format required by the
practice setting. 3. Professional Behavior/Customer Service •

therapy services. (eg, critical/clinical pathways, protocals) •
Participates in Performance Assessment and Improvement program. 5.
Professional Development • Maintains knowledge of current trends
and techniques by attending related conferences/seminars, reading
appropriate journals, books etc. Participates in inter and
intra-departmental instruction. • Promotes the profession of
Physical Therapy. • Willingness to take on and develop new projects
and services. • Takes part in departmental program planning and
development. • Develops and implements plans for self-improvement.
Minimum Qualifications: To qualify you must have a Graduation from
an approved Physical Therapy curriculum with a minimum of a
Bachelor of Science Degree. New York State Physical Therapist
license or eligible for licensure with current registration.
Demonstrates professional behavior during interaction with others.
Adapts delivery of physical therapy care to reflect respect for and
sensitivity to individual differences. Communicates in ways that
are congruent with situational needs. Ability to walk and stand for
up to 75% of work time and lift and turn patients. Ability to focus
on patient needs and detail 75% of work time. Practices in a safe
manner that minimizes risk to patient, self and others Current
