Registered Nurse (RN) | Home Health Location: Long Island City, NY Agency: Cynet Health Pay: $2,198 per week Shift Information: Days - 5 days Contract Duration: 260 Weeks Start Date: ASAP About the Position TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Long Island City, New York, 11102! Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 260 Days Shift: 5x5 Days Hours per Shift: Standard Nursing Hours Experience: Two years of experience in a home health setting within the last ten years. License: Current RN Licensure in New York State Certifications: BLS (required before the start date) Must-Have: - Two years of experience in a home setting, which may include working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. - Experience conducting comprehensive assessments of patients. - Experience caring for a minimum of two patients per week. Description: Required Licensure: Current RN Licensure in New York State. Required Experience: Two years of experience in a home setting within the last ten years. Nurses must have recent experience conducting comprehensive assessments of patients. All nurse assessors must meet specific requirements, including two years of experience in a home setting. Home care experience can include working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Providing primary care to homebound individuals is also relevant. Experience with care management for individuals receiving home and community-based services, including hospice and assisted living, is preferred. Candidates should have experience caring for a geriatric population and managing chronic diseases in a home setting. Experience that does not satisfy the required qualifications on its own includes private duty nursing and case management limited to utilization reviews. Job Summary: The ability to review health status questions and make evaluations based on specific criteria for enrollment is essential. Working with technology in the workplace is necessary. Knowledge of the theory, principles, and practices of general nursing is required. Understanding state and agency laws and regulations is important for compliance. Excellent organizational, interpersonal, written, and verbal communication skills are necessary. Candidates must demonstrate the ability to communicate effectively with various consumers. Sensitivity to the concerns of others is a must. The ability to perform in a fast-paced, deadline-oriented environment is required. Candidates should be able to manage complex tasks simultaneously and work independently. Job Responsibilities: Perform initial in-home pre-evaluations of patients, including health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms relevant to cases. Enter evaluation data into electronic forms and transmit as required. Document any concerns and issues that arise during evaluations. Emphasize continuity of care to reduce gaps in treatment plans. Discuss healthcare options, supports needed, and service vendor options with consumers. Communicate with eligible consumers for enrollment into managed care health plans. Enhance communication and collaborative relationships with interdisciplinary care team members. Collect quality review data and document required information for outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain a comprehensive working knowledge of community resources and payor requirements. Meet established performance standards for the position.
