Description for Nurse Practitioner ( NP ) for our Long Term Care
Facilities ( LTC ); We are seeking a dedicated and compassionate
Nurse Practitioner (NP) to provide bedside patient care at our
Long-Term Care (LTC) facilities in Wall Township, NJ. This is a
full-time, in-person role, Monday through Friday, from 9:00 AM to
5:00 PM. Qualifications of the Nurse Practitioner ( NP ) for our
Long Term Care Facilities ( LTC ): Valid Nurse Practitioner ( NP )
license in the state of New Jersey Responsibilities of the Nurse
Practitioner ( NP for our Long Term Care Facilities ( LTC ): Upon
admission to the facility, examine residents and their medical
records. Check ups, management of chronic conditions and
medications, transitional care management, discharge planning and
