Job Description LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Morristown, New Jersey. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 04/20/2026 - Duration: 13 weeks - 37.5 hours per week - Shift: 8 hours, days, nights - Employment Type: Travel Facility is seeking an experienced RN Case Manager to coordinate patient care throughout acute hospitalization and ensure safe, efficient discharge planning and transitions of care. The Case Manager will collaborate with interdisciplinary teams to manage utilization review, quality screenings, and patient discharge processes. This role requires strong inpatient case management experience and the ability to function effectively in a fast-paced acute care setting Key Responsibilities - Coordinate patient care throughout acute hospitalization - Manage all aspects of discharge planning and care transitions - Perform utilization management and quality screenings - Collaborate with physicians, nurses, and interdisciplinary teams - Ensure appropriate level of care and timely discharge - Facilitate communication between patients, families, and healthcare providers Required Qualifications - BSN (Required) - Active RN License (Required) - Minimum 2 years of acute care experience (strongly preferred) - Inpatient Case Management experience (Required) Preferred Qualifications - CCMC (Certified Case Manager) Certification About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
