Job Description:
A company is looking for a Member Care Coordinator for their
remote, full-time position in the operations department.
Key Responsibilities:
Facilitate enrollment and discharges into and out of the
organization
Manage clinical care transitions and coordinate mental health
services
Develop relationships with outside service providers and ensure
compliance with care coordination
Required Qualifications:
Master's degree in a related field
Unrestricted state licensure preferred (LCSW or LPC)
2-3 years of post-licensure experience
Experience in care coordination or case management in a mental
health or behavioral health setting
Bilingual English/Spanish preferred
