employment JOB SUMMARY: To promote the mission of PATH to help
individuals achieve a more independent and fulfilling life by being
an effective team member. Work primarily with adults with serious
mental illness or emotional disorders, assisting individuals to
make full use of Behavioral Health Services and natural community
supports to achieve a more independent and fulfilling life.
Services provided will include developing a partnership with the
individual to assess needs, obtain services, and resolve crises.
QUALIFICATIONS: Bachelor’s Degree in Human Services field from an
accredited college/university One to two years experience in the
behavioral health field Valid driver’s license required Experience
working with adults Good interpersonal and organizational skills
Ability to use electronic health record required SPECIFIC DUTIES:
Develop partnerships with assigned individuals to assess needs,
obtain services and resolve crises Assist individuals with
identifying goals and defining the steps needed to achieve their
goals Complete all assessment and planning documentation including
Adult Psychosocial Assessment; Strengths Assessment; Treatment
Plans and other required documentation Ensure all required
documentation is completed in a timely, comprehensive, and legible
manner and is in compliance with PATH policies and procedures
Assist in linking individuals with their chosen services and
resources in the community such as needed psychiatric and medical
treatment, residential services, financial entitlements,
social/recreational activities, and general social services We are
